Annual Physical Exam
Physicals are recommended yearly. Your nurse will send home a form at the end of each school year. If your doctor completes a physical form, please return it to your nurse.

Immunization Requirements
Please see the following for immunization requirements for school attendance in New Jersey.
Vaccines at 11 Years Old
DTaP and Meningococcal (Brand name known as: Menactra) Immunizations:
- Any student entering grade 6 or age 11 years must receive an updated dose of each vaccine.
Students who do not comply will be excluded from school until requirement is met.
Please make sure your child's physician fills in the following vaccines if your child is 11 years old or is entering the 6th grade.
